This Privacy Policy for OneVeracity, constituting OneVeracity, LLC and its subsidiaries and affiliates (including Veracity Analytics, Inc., Veracity OnSite, LLC, Maple Leaf, LLC, Veracity Care Solutions, LLC, and Anchor Benefit Consulting, LLC ("Anchor")) (collectively, "OneVeracity," "Company," "we," "us," "our"), describes how and why we might access, collect, store, use, and/or share ("process") your personal information when you use our services ("Services"), including when you:

Information automatically collected

In order to enhance our ability to provide relevant marketing, offers, and services to you and update our records, we may obtain information about you from other sources, such as public databases, joint marketing partners, affiliate programs, data providers, and other third parties. This information includes mailing addresses, job titles, email addresses, phone numbers, intent data, IP addresses, social media profiles and URLs, and custom profiles, for purposes of targeted advertising and event promotion.

We may also process your information for other purposes with your consent, including to respond to user inquiries and offer support to users.

  • OneVeracity Companies. We may share or transfer information between companies of OneVeracity, including Anchor, to provide the Services.
  • Members and Service Providers. We may share or transfer information to our members and third party service providers necessary to provide the employee benefits and other administrative services pursuant to contracts with such members and third party service providers.
  • Law Enforcement, regulators, government agencies, courts or other third parties. We share with these parties if legally required or allowed.
  • Business Transfers. We may share or transfer your information in connection with, or during negotiations of, any merger, sale of company assets, financing, or acquisition of all or a portion of our business to another company.

We may also share information with third parties not listed with your consent.

Google Analytics. We may share information on your interactions with public-facing pages of the Sites with Google Analytics to track and analyze use of the Services. We do not use or provide information to Google Analytics in connection with parts of the Sites where health information is transmitted or shared, or where such transfer would be prohibited by applicable law. When we have no ongoing legitimate business need to process your personal information, we will delete or anonymize it, or, if that is not possible, securely store it and isolate it from further processing until deletion is possible.

In short: We do not knowingly market to children under 18. We may collect data from minors aged 13 to 17 where applicable legal requirements permit such minors independent legal authority to control access to their health records. By using the Services, you represent that you are at least 18, or at least 13 with legal authority under federal or state law to access health information and related features. If you are a participant in an employee benefit plan, OneVeracity — including Anchor in its role as a third-party administrator — may provide services to your employer, including processing health information relating to such employer's benefit plan offerings. In such instances the employer controls the use of your personal information and we collect such information to provide services pursuant to our agreement(s) with the employer. We receive instructions from your employer and are bound by these instructions via our agreements with your employer. This information may also be subject to state and federal laws, including HIPAA. To the extent HIPAA applies, its requirements supersede this Privacy Policy and we will comply with HIPAA.

Most web browsers and some mobile operating systems include a Do-Not-Track ("DNT") feature. Because there is no uniform technology standard for recognizing DNT signals, we do not currently respond to them. California law requires us to disclose this.

We have collected the following categories of personal information in the past twelve (12) months: identifiers (contact details, IP address); California Customer Records data (name, contact information, employment, financial information); protected classification characteristics (gender, age, race, national origin, marital status); and sensitive personal information (health data). We do not collect commercial information, biometric information, internet/network activity, geolocation data, audio/visual information, professional/employment information (beyond the above), education information, or inferences for profiling.

We have not disclosed, sold, or shared any personal information to third parties for a business or commercial purpose in the preceding twelve (12) months, and will not do so in the future.

California Civil Code Section 1798.83 permits California residents to request, once a year and free of charge, information about categories of personal information we disclosed to third parties for direct marketing purposes in the preceding calendar year. We value your privacy. Any personal data collected through SMS or text, including mobile phone numbers and message content, will be used solely for communication related to our services and will never be shared, sold, or disclosed to third parties, except as required by law.
